• IMAP at Bowling Green State University Request Form

    This request form takes approximately 5-7 minutes to complete.
  • I understand:

    • If approved for enrollment, as a State of Ohio designated IMAP provider, BGSU will deliver IMAP training(s) to me at no cost.
    • Trainers will review all requests for enrollment. A personal interview with a trainer may be requested by the trainer or the enrollee.
    • BGSU will only collect and use my personal information for the limited purpose of IMAP grant reporting to the State of Ohio and will protect my personal information. 
    • In this application, I must provide certain personal information for grant reporting purposes, including:
      • Gender
      • Race
      • Last 4 digits of my social security number
      • Birthdate
      • Address
      • E-mail
      • Telephone number

    You will receive a final acceptance e-mail officially enrolling you in your requested IMAP training(s) at BGSU within 5-10 business days following your request for enrollment or following your telephone interview, if requested by yourself or a BGSU Trainer.

  • I understand that if I am approved for one or more IMAP trainings, I am taking a seat in a training that could be filled by another eligible individual. Therefore, I commit to:

    • Completing my training(s) during the applicable 4-week window.
    • Completing my training(s) by actively participating in all training activities and completing all associated assignments and assessments.
    • Supporting BGSU in its reporting obligations to the State of Ohio, by completing all fields in this request form and providing BGSU with the personal information described above.
    • Timely communication with your trainer and IMAP Office at BGSU, if any exceptional circumstances arise that would cause me to not complete my training in the advertised 4-week window.”
  • The following trainings are all online and self-paced, however they must be completed within the designated 4-week window. Each training allows for you to interact with a BGSU trainer, if desired. Each training is estimated to take 16 hours to complete. Therefore, you should plan to spend about 4 hours each week completing your training.

    • Manufacturing Process Control: TRAINING FULL

  • Hybrid Trainings

  • Hybrid trainings are delivered partially online and also require in-person attendance at the BGSU Main Campus. Your selected hybrid trainings are an on-campus requirement as listed below. All online work MUST be completed before you attend the on-campus portion of your training. Failure to complete your online training before your in-person attendance may result in removal from the training and ineligibility to participate in future IMAP offerings at BGSU.

    • FANUC Robot Operations, Required on Campus Day: January 5, 2026
    • CAM Manufacturing Fundamentals, Required on Campus Day: February 2, 2026
    • Sensors and Vision Systems, Required on Campus Day: March 2, 2026
    • Additive Manufacturing, Required on Campus Day: May 4, 2026
